Program Description and Application Guidelines

The Trusts make a small number of grants each year to non-profit, community-based health centers located within the District of Columbia that provide primary and specialty care to uninsured, underinsured, or low income individuals and families for whom access to basic health care is limited. Health care may include physical, dental, and mental health services. Health services may include such specialties as pre-natal and well-baby care, women's reproductive health, and mobile health vans, as well as the full range of services necessary for children, youth, adult, and elder care.

Grants may be requested for general operating support, implementation of new initiatives or expansion of existing programs. The Trusts are especially interested in requests from community-based clinics that have received or are moving toward becoming a Federally Qualified Health Center (FQHC) or similar designation, and those participating in the Medical Homes DC initiative of the DC Primary Care Association (DCPCA).

As a member of the Washington AIDS Partnership, the Prince Charitable Trusts does not consider requests for health care services to persons who are HIV+ or needing AIDS care. Requests for these services should be directed to the Washington AIDS Partnership.

Organizations that are applying for support under the Trusts' Hospital Morale program may not simultaneously apply for a grant under the Health: Community-Based Health Clinics Program.
