Eligible nonprofit organizations may apply for
Foundation grants by submitting full proposals.
Please follow the instructions below.
Limit your proposal to five typewritten pages,
double spaced, plus attachments. Please do not
staple the proposal or any of its sections. Do not
attempt to hand-deliver applications without an
appointment as the building is locked at all hours. Applications may be submitted by mail only (we no longer accept proposals via email). Deadlines are post-mark deadlines, not delivery deadlines.

Grantmaking Calendar
Deadline for submission of 2010 proposals: Tuesday, September 1, 2009.
Awards are announced in December.
Grant checks are disbursed in January.
Disbursement and reporting: Grants at or
below $10,000 are disbursed in one check upon
award; a final report is expected at the end of the
grant year. Grants greater than $10,000 and less
than $80,000 are disbursed in two checks: upon
award, the first check will be for 6 months. Upon
receipt of an acceptable midyear report, a check
for the remaining 6 months will be issued. Grants
greater than $80,000 are disbursed quarterly and
reports are due quarterly. Report format and due
dates are provided upon reward.
Proposal Narrative
BACKGROUND: Describe your organization’s mission,
history, overall goals and/or objectives. Describe
your current programs and activities. List the
organization’s major accomplishments from
the past two years.
ACTIVITY PROPOSED: Title of project or program;
location of proposed project; amount requested.
PROJECT DESCRIPTION: Identify the need, target
population, overall goals, measurable objectives,
intervention strategies, and the capacity of project
agencies/staff to meet project goals and objectives.
Include timeline for duration of project (include
expected start and completion dates). Please specify
how proposed activities relate to the Ravenswood
Community Health Needs Assessment.
EVALUATION: Explain program evaluation
method(s) and implementation plan.
FUNDING: List current and/or potential sources
of funding. Explain how the program will be
supported once the grant period ends.
PROGRAMMATIC AND FISCAL OVERSIGHT:
Identify staffing and qualifications, supervision
(if applicable), and financial management.

Nonprofit Organizations
- Cover letter on agency letterhead
- Proposed program/project budget
(income and expenses)
- Operating budget for the current fiscal year
- Letter from the IRS determining applicant's tax-exempt status
- Board of Directors list, with affiliations
- Audited financial statements for previous fiscal year and a copy of the organization's most recent Form 990 filed with the IRS
- If project is working with area schools, health
care providers, or other nonprofit organizations,
include letters of support
- List of foundations, corporations and governmental
agencies that funded the organization
in the past fiscal year, including amounts
contributed ($1000 and above)
Advocate Health Care Programs
- Cover letter on department letterhead
- Program or project budget
- Identify who will provide fiscal
oversight/management
- List of support or sponsors from other sources,
including hospital, if applicable
