History and Purpose

The current Ravenswood Health Care Foundation giving program arose out of the merger of the former Ravenswood Medical Center into Advocate Health Care. In 1999, Advocate began to contribute $1 million per year to the Foundation. Although the Medical Center has since closed, the Foundation will continue to benefit the Ravenswood and surrounding communities. The Foundation will grant $1 million per year through 2017. The mission of the Ravenswood Health Care Foundation is to award grants that foster, promote, support, develop and encourage community health and wellness in the greater Ravenswood area of Chicago.

Areas of Interest

The Foundation strives to support and improve the health and wellness of individuals and communities within the greater Ravenswood area.We interpret “health” to refer to direct medical care, while “wellness” refers to health education and disease or illness prevention efforts.

Primary — though not exclusive — areas of interest are identified in the Ravenswood Community Health Needs Assessment (2002). The Foundation will periodically conduct follow-up needs assessments.

Types of Support

The Foundation accepts proposals for new as well as existing projects and programs.While a greater risk exists in supporting start-up organizations, the Foundation is willing to consider such proposals. The Foundation prefers not to be the sole funder of any one organization or program; however, the Foundation will consider supporting new activities that show promise for broader support. Grants are awarded for one year; the Foundation does not make multi-year commitments. Applicants may apply for continued support of a previously funded project. Rarely, however, will an agency receive funding for the same purpose or project for more than three years.

Eligibility

The Foundation supports only organizations that are exempt from federal income taxation and are described in Section 501(c)(3) of the Internal Revenue Code, as well as consistent with and complementary to the mission and charitable, tax-exempt purposes of Advocate Health Care. Grants are not made to individuals, businesses, or private providers. Grants are awarded for health and wellness activities that take place in the greater Ravenswood area only. The greater Ravenswood area is defined as the primary service area of the former Ravenswood Medical Center, which encompasses the following zip code areas:
