Grant recipients must submit reports at 6 months and at the end of the grant year:
- Mid-year reports are due no later than July 30, 2011
- Year-end reports are due no later than January 31, 2012
Please note: The release of the second half of a grant is contingent upon receipt of an acceptable mid-year report.
Grant reports should include the following information, and may be submitted either by postal mail or email. Be sure to date the report and include a contact person and phone number.
- Title of project or program and date of grant award.
- List the objectives of the project and briefly describe the extent to which each objective was met; provide concrete examples of accomplishments and outcomes.
- If there is any need to change or alter objectives, specify new objectives and the reason(s) for the change.
- If any obstacles or challenges were met and/or overcome during the course of implementation, please describe what was learned in the process.
- If this is a Mid-Year report, identify activities that are projected for the next reporting period. If this is a final report, identify any plans for the future of the program.
- Include a budget report identifying grant expenditures.
The deadline for submitting 2012 proposals is Thursday, September 1 2011.
